The Restaurant Reservation System Breaking Down
Imagine muscle, fat, and liver cells are restaurants, and insulin is the reservation confirmation message. Normally, insulin texts each restaurant saying "table ready for glucose," and the maitre d' (insulin receptor) lets customers in. In Type 2 diabetes, two things go wrong together: first, the restaurants stop trusting the text messages (insulin resistance)—their maitre d's get distracted by inflammatory noise from the kitchen (cytokines from belly fat), so diners pile up outside. Second, the reservation center (pancreas) tries to send triple the messages to force restaurants to accept diners, but after years of this panic-level texting, the staff burns out and quits (beta-cell exhaustion). Meanwhile, the main supply warehouse (liver) ignores the "stop sending food" message and keeps dumping glucose onto the street outside, making the crowd of stranded diners bigger and bigger.
