The Burglar in the Safe Who Hides from Guards

M. tuberculosis is like a burglar with a diamond (the bacteria's mycolic acid coat) that lets it survive inside a safe (the macrophage) even after the safe's acid spray and poison gas (lysosomes and enzymes) are triggered. The body's security team (T cells) notices the burglar is in the safe and builds a concrete vault around it (granuloma), which stops the burglar from robbing the rest of the building but does not remove the burglar—he just sits there, barely moving, sometimes taking a tiny step. As long as the guards stay alert and strong (good CD4+ count and cell-mediated immunity), the vault holds and the burglar stays trapped. But if the guards get tired, distracted, or fewer in number (HIV, steroids, diabetes, old age), the vault cracks, the burglar escapes, multiplies into a gang, destroys rooms (lung cavities), and spreads chaos throughout the building (disseminated TB and meningitis). The tragedy is that the burglar was never truly locked in—just contained by constant guard effort.
