A Highway After a Crash

Think of the bowel as a highway where traffic (food, liquid, gas) normally flows smoothly downstream because traffic controllers (the vagus nerve and enteric nervous system) keep the lights green and the lanes clear. Surgery is like a major accident: it damages the road (bowel wall inflammation), injures the traffic controllers themselves (nerve stunning), and floods the area with emergency responders (inflammatory cytokines) who are so busy that they accidentally shut down the control tower (suppress parasympathetic signaling). Opioid pain pills are like a new law that forces every vehicle on the road to go into neutral and coast — they jam the traffic system at the molecular level, not because the road is blocked, but because the vehicles cannot accelerate. When traffic backs up, pressure builds on the road (distension), which triggers the emergency broadcast system to tell drivers to stop (the intestino-intestinal reflex), making the jam worse. The longer traffic sits still, the more exhaust builds up (bacterial overgrowth), the road surface cracks from pressure (ischemia and barrier breakdown), and the risk of a multi-vehicle pileup or environmental disaster rises (perforation or translocation).
