A Clogged Drain Backing Up a City

Imagine the liver is a massive water treatment plant, and the portal vein is the main pipe bringing dirty water from all the neighborhoods (intestines, spleen, pancreas, stomach). Normally, water flows through the plant's filter beds (sinusoids) smoothly at low pressure, gets cleaned, and exits through the outgoing pipe (hepatic vein) to the river (heart). When the filter beds get clogged with years of sludge and hardened deposits (cirrhosis/scar tissue), water can't flow through anymore — pressure builds up in the incoming pipe like a backed-up drain. The city then opens emergency overflow channels (collateral veins) through the neighborhoods, but these are old, fragile pipes never designed for high pressure — they bulge and crack (varices form). Meanwhile, the neighborhoods' arteries sense the backup and dilate wider (splanchnic vasodilation), trying to force more water in, which makes the jam worse. When one of those emergency pipes finally ruptures, water floods everywhere uncontrolled (variceal bleeding), and without emergency repairs and draining (endoscopy, octreotide, blood transfusion), the whole system drowns.
