The Gas Tank After the Pump Breaks

A newborn's body is like a car that just switched from a gas station attendant (the mother, pumping fuel constantly through the umbilical cord) to relying on its own fuel tank and engine. The liver is the gas tank — it stores gasoline as glycogen and releases it as glucose (the actual fuel molecules) when the engine (brain, heart, muscles) needs power. The pancreas is like the fuel gauge and automatic pump: it senses how much fuel is in the blood and releases insulin (which pushes fuel into cells) or glucagon (which pulls fuel from storage). When the umbilical cord is cut, the tank should start draining just enough to keep the engine running, and the backup pumps (glucagon, epinephrine, cortisol, growth hormone) should kick in to release more fuel. But if the tank is empty (premature baby with no glycogen stored), or the fuel pump is stuck in the 'pump in' mode (too much insulin in an infant of a diabetic mother), or the engine is suddenly running at redline (a cold baby shivering or a baby fighting infection burns fuel 10 times faster), the blood fuel level crashes — and the engine (brain) starves in minutes.
