Dam Overflowing and Cracking

Your small blood vessels are like a dam with gates (arteriolar smooth muscle) that open and close to regulate water flow downstream. The gate operators (endothelial cells) can sense the water pressure and adjust the gates to keep flow steady — this is autoregulation, and it normally works even when upstream pressure swells from rain (normal blood pressure swings). But in with high blood pressure (hypertensive) emergency, the upstream pressure becomes so extreme so fast that the gates jam shut, cannot hold the force, and the dam itself starts to crack (fibrinoid tissue death (necrosis) of the vessel wall). Water (blood) and debris (plasma proteins, red cells) leak through the cracks into the surrounding ground (organ tissue), the dam develops structural cracks that can burst outright (intracerebral bleeding (hemorrhage)), and the soil downstream dries out (ischemic stroke from vessel spasm and clotting). The organs fed by that water — the power plant (heart), the control center (brain), the filtration plant (kidneys), and the security camera (eye) — all start to fail because the steady flow they depend on is gone, replaced by flooding in some places and drought in others.
