A Broken Smoke Alarm in a House Fire
The placenta is like a smoke detector that's supposed to give a gentle warning beep when it detects pregnancy hormones (the smoke). In normal pregnancy, the beep (mild nausea) tells mom to be cautious about food — a useful safety feature that fades by 14 weeks. In hyperemesis gravidarum, the detector is broken and screaming at maximum volume nonstop, even though there's no real fire — the mom's brainstem vomiting center (the alarm's speaker) is stuck on because hCG, estrogen, and placental GDF15 are flooding the chemoreceptor trigger zone with an overwhelming signal. She vomits constantly, can't eat or drink, and her body bleeds out water, sodium, and potassium with each retch — like the house slowly burning. Without intervention (turning off the alarm with antiemetics and IV fluids + thiamine), her electrolytes crash, her brain starves for thiamine, and if glucose is poured in without thiamine first, the brain cells die within hours, like flashover destroying everything inside.
