The Overworked Sponge That Forgot How to Open

Imagine a kitchen sponge that has been squeezed against a hard countertop every single day for years. The sponge's fibers get compressed and matted down (this is the left ventricle's wall thickening). One day you try to soak it under running water, but it no longer opens up and absorbs water the way it used to—the fibers stay bunched and dense (diastolic dysfunction: it cannot relax). Meanwhile, the water pipes in your house have also calcified and hardened from mineral buildup (arterial stiffening), so the water pressure backs up instead of flowing smoothly. You keep turning up the faucet harder and holding the sponge under longer to force water in (compensation: higher blood volume and sympathetic drive), but the sponge still will not expand, and now water is pooling and leaking all over the counter (pulmonary swelling (edema) and peripheral swelling). The sponge can still squeeze out the water it holds (ejection fraction is normal), but it cannot fill anymore, and that is the real problem.
