The One-Line Truth
The whole disease in one chain

The penis cannot get hard enough for sex or stay hard long enough to finish.
Erection as a Water Balloon Inflation System
Imagine a water balloon (the penis) with two main tubes running through it—one inlet hose (artery) brings water in, one drain hose (vein) lets water out. The balloon sits inside a tight rubber bag (tunica albuginea) that cinches when the balloon swells. Your brain is the operator: when it sees something exciting, it flips a switch that tells a valve (the endothelium) to open the inlet hose wide and squeeze the drain hose shut. Water rushes in, the balloon inflates, the rubber bag tightens and traps the water inside, and the balloon stays firm. But if the inlet hose gets clogged with rust (atherosclerosis), the valve sticks and won't open (endothelial dysfunction), the drain hose leaks (venous leak), or the brain never sends the signal (nerve or psychological break), the balloon stays floppy no matter how hard you try. The system works because every single step—signal, valve opening, water flow, and drain closure—must happen in sequence; break one and the whole mechanism fails.
