The kidneys have been losing their cleaning power slowly, over months or years, and they cannot heal back.

A factory losing its workers and machines

Imagine a water treatment plant with 1 million workers (nephrons) filtering 50 gallons per second. Diabetes and high blood pressure are like toxic fumes that kill workers slowly—they don't come back. As workers die, the survivors work twice as hard to keep up (hyperfiltration), but this exhaustion kills them faster, creating a downward spiral. Meanwhile, the plant loses its ability to adjust water flow (fluid control), regulate mineral balance (phosphate and calcium), and make the chemical signals (erythropoietin) that tell the body's bone marrow factory to keep making red blood cells. The remaining workers can't remove the acid waste either, so it pools up in the system. Eventually, the plant can't keep the body's internal environment stable, and the whole system fails without outside help (dialysis or transplant).
