Antibiotic Carpet Bombing and the Awakening

Your colon is like an apartment building where good-tenant bacteria pay rent by keeping the building clean, safe, and fully occupied. C. difficile spores are like squatters sleeping in the walls — they can't move in as long as every room is full and the good tenants control the environment. Antibiotics are like a carpet bomb that kills most tenants indiscriminately (especially certain strong ones like fluoroquinolones and clindamycin). With 90% of the good tenants dead, the squatters wake up, multiply fast, and start spraying toxic chemicals (toxins A and B) that punch holes in the walls (the colon lining cells). The building's immune system (white blood cells) rushes in to fight, but if toxins destroy the walls faster than repairs happen, the whole structure weakens — rooms flood with water (diarrhea), the frame warps dangerously (toxic megacolon), or the walls tear open (perforation), spilling sewage into the street (septic shock).
