Benign prostatic hyperplasia (non-cancerous prostate growth) squeezes the urethra (the tube that drains urine from the bladder), blocking urine flow and causing both trouble emptying (weak stream, hesitancy) and trouble storing (urgency, frequency, waking at night).

A Clogged Pipe and the Struggling Pump

Imagine a basement sump pump (the detrusor muscle) that drains water (urine) through a pipe (the urethra) surrounded by expanding foam insulation (the growing prostate). Early on, the pump works harder and its motor (muscle fibers) grows bulkier to push water through the tightening pipe—for a while it keeps up, so you barely notice the problem. But the harder the pump strains against the narrowing, the less blood reaches its worn-out motor (ischemia), and the electrical signals that trigger pumping get scrambled by the stress (nerve damage). Eventually, the pump misfires randomly—twitching even when the tank is not full (detrusor overactivity)—and then loses its squeeze power entirely (decompensation), so water pools in the tank and backs up toward the foundation (hydroureteronephrosis). The stagnant water becomes a swamp for algae and rust (bacteria and stones). The whole cascade starts with one break: the pipe got squeezed.
