Phase 1 · Understand the Body — Why the disease behaves the way it does.
The One-Line Truth
The whole disease in one chain
Everything on this page exists to explain this line.
Mania is a medical emergency when the person cannot think clearly, acts dangerously, or loses touch with reality.
A car with a stuck accelerator and failed brakes
Imagine a sports car where the gas pedal (dopamine) gets jammed wide open and the brake system (prefrontal cortex and GABA) fails simultaneously. The engine (mesolimbic reward circuit) roars uncontrollably, flooding the driver with the thrill of speed—this is the euphoria and grandiosity of mania. The driver feels invincible and brilliant, so they refuse to pull over for help. Meanwhile, the sleep center is a pressure gauge that normally rises as the day goes on; high dopamine physically breaks this gauge, so the driver never gets the signal to rest, even though the engine is burning fuel at triple speed. The steering wheel (executive judgment) won't respond to turns—every impulse feels like the right direction, so the driver swerves into dangerous territory (risky spending, confrontations, hypersexuality) without sensing the cliff ahead. After days of driving flat-out, the fuel tank empties, the engine seizes, and the driver crashes into a ditch (the post-manic depressive crash with suicidal risk) surrounded by wreckage they caused but cannot now remember choosing.
