Alcohol Withdrawal & Delirium Tremens

The one-line truth

Delirium tremens is a brain emergency that strikes 2 to 4 days after the last drink, marked by severe confusion, seeing or hearing things that are not there, and an autonomic storm — the body's normal control over heart rate, blood pressure, and temperature collapses.

The Thermostat Overshoot After the Furnace Shuts Off

Imagine a house's heating system has been running for months with the thermostat set to 68°F, but the furnace is broken and stuck half-on. The house's walls and insulation have adapted by thickening (like the brain removing GABA receptors) and the air vents have shrunk (like adding NMDA receptors) so the room stays 68°F anyway despite the broken furnace. Now someone suddenly flips the furnace completely off. The thick insulation and shrunken vents were designed to work with the half-on furnace; without it, the room's temperature control system overshoots wildly. The thermostat keeps firing the furnace on (but it's off) and the house's emergency heating coils (sympathetic nervous system) kick in at maximum, the vents slam open, and every temperature sensor in the walls misfires. The house starts shaking from the stress, sweating forms on the windows, and pipes rattle. The thermostat is not broken — it is tuned to the wrong situation. That is delirium tremens: the brain's control system was built for alcohol-on and suddenly faces alcohol-off, so every signal overshoots.
